Note: I do not submit claims to health insurance. Paid receipts for you to submit to your insurance for reimbursement will be provided. Payment is required at time of service. Check with your insurance company to see if they provide reimbursement for massage therapy prior to your first appointment.
A physician's referral is required for all motor vehicle accident, workman's comp, and personal injury clients.
Health Savings Account (HSA) cards are accepted. Please inquire prior to your first appointment.
I do not bill health insurance.
